YOUR SHARE THE CARE JOURNEY GUIDE (or ROAD MAP) This reference road map can help group members easily
locate information they may need on key subjects covered in the Share The Care book. Though the beginning chapters of Share The Care are written to the two people who will organize
the first meeting, everyone should read the book as it provides
valuable (individual and group) advice and ideas to help them through
stressful times or situations they may encounter during their journey
together. All of our Share The Care forms have been incorporated
into this site. They are also still
accessible for download on www.sharethecare.org. Please note, we
have replaced the Medical History Form (in the book) with two new online
forms: the Medical Profile and the Medical Events Log. |
